Homeless Leasing Assistance Program
To date, Community Housing Network, Inc. has been awarded four grants in Macomb County through the HUD, Supported Housing Program. We provide leasing assistance to 48 formerly homeless individuals and/or families in 34 housing units within Macomb County. CHN also offers program specific supportive services to heads of households having severe and persistent mental illness, developmental disabilities, and/or substance abuse problems. A CHN Housing Navigator/Supportive Specialist provides these services which may include Outreach, Housing Placement, Case Management, Life Skills Instruction and Housing Education. CHN also works in close collaboration with Macomb County Community Mental Health for other Medicaid funded supportive services, including case management and mental health counseling.
Each potential program participant must be receiving services from Macomb County Community Mental Health Authority and must be referred by their case manager. Eligibility requirements include written documentation to verify the head of household is disabled due to a severe and persistent mental illness and/or developmental disability, and is homelessness. Verification of disability status must come from a qualified source, such as a physician or Core Provider. Individuals/families that are chronically homeless will receive priority in the selection process. A chronically homeless person is defined as having been homeless for a year or as having received homeless provider services at least four times in the past three years. Documentation of chronic homelessness will be required.
