Additional Definitions
Advocate is a person who will argue for a cause, or plead on another's behalf for education, legal or vocational rights; or a person who argues for their own, or another person's rights.
Case Manager is a person who assesses, plans, implements, coordinates, monitors, and evaluates the options and services required to meet an individual's mental health and health needs to promote quality, cost effective outcomes.
Criteria A standard on which a judgment or decision may be based.
Medicaid A jointly funded, federal state health coverage program for certain low income people.
Medicare A federal health insurance program for people age 65 and over.
Social Worker Advocates and arranges post hospital or post rehabilitation services and programs for persons. Also provides counseling with consumers and their families.
PCP The core function of the case manager, social worker, and support staff is to uphold the person centered plan.
