Shelter Plus Care

Community Housing Network administers one HUD sponsored Shelter Plus Care grant in Oakland County.  The program provides 35 units of permanent supportive housing through the leasing of scattered site rental units. A total of 55 formerly homeless persons with mental illness (including family members) are served through this program.  Community Housing Network provides assistance to participants in identifying housing units and supportive services.  The services are offered by the Oakland County Community Mental Health Authority core providers and other mainstream resources.  Some services may include case management, housing services, transportation, life skills training, and mental health counseling services following a Person Centered Plan.  For more information about the CHN Shelter Plus Care program in Oakland County, please contact the Housing Navigator at 248-928-0111.
