Permanent Supported Housing Homeless Housing Leasing Assistance Programs
Community Housing Network, Inc. is pleased to have been awarded several grants through the HUD, Supported Housing Program. The Leasing Assistance Programs provide rental assistance and program specific supportive services to persons and/or families meeting the program requirements of having a Severe and persistent mental illness and/or developmental disability and are currently homeless. A CHN Housing Navigator/Supportive Specialist will provide program specific supportive services which may include Outreach, Housing Placement, Case Management, Life Skills Instruction and Housing Education and work in close collaboration with Oakland County Community Mental Health Authority's Core Providers for other Medicaid funded supportive services, including case management and mental health counseling.
Program Eligibility Requirements
In all cases, written documentation must be obtained to verify the applicant is disabled due to a severe and persistent mental illness and/or developmental disability, and is homelessness. In families, the head of household must be the person with the qualifying disability. Verification of disability status must come from a qualified source, such as a physician or Core Provider. Individuals/families that are chronically homeless will receive priority in the selection process. A chronically homeless person is defined as having been homeless for a year or as having received homeless provider services at least four times in the past three years. Documentation of chronic homelessness will be required.
