Support coordination
The Oakland County Community Mental Health Authority (OCCMHA), with offices located in both Pontiac and Southfield, provides high-quality outpatient services to adults challenged by developmental disabilities.

To be eligible for this program, a person must be an adult living in Oakland County who has been diagnosed as having a developmental disability. This is defined as a severe or chronic disability that is attributable to mental and/or physical impairment and is likely to continue indefinitely. This condition should be: evident before and individual reaches that age of twenty-two; cause the individual significant restriction in at least three key life activities (self-care, use of receptive and/or expressive language, learning mobility, capacity for independent living, or economic self-sufficiency); and reflect a person's need for an individually planned and coordinated program of care on a long-term, even lifelong basis.
The OCCMHA offers the following services:
Providing counseling and behavioral care
Establishing a legal guardian if needed
Educating consumers about human sexuality
Providing access to community housing
Linking consumers with services by working with support coordinators
Helping consumers to find jobs/day programs
Providing and linking to individualized health services
Providing and/or linking additional specialty services/supports
Services for Adults with Developmental Disabilities clinical and support coordination staff are community and agency based. Normal agency office hours are Monday through Friday form 8:30 a.m. to 5:00 p.m. Evening appointments occur regularly by appointment.

Support Coordinator is a relatively new term and a new Habilitation Supports Waiver (HAB) coverage which replaces "case manager" and "client services manager", and applies to persons who deliver such services to adults with developmental disabilities. However, the terms are still often interchanged. The function of the support coordinator incorporate the core elements of case management as outlined in Medicaid chapter 3: assessment, support plan development, linking, coordinating and monitoring of supports and services and reassessment and follow-up.
The role/function of the Support Coordinator/Case Manager is identified in the consumer's person centered plan. The Support Coordinator/Case manager is responsible for assuring that all activities outlined the plan are implemented unless otherwise indicated.
The following tasks and activities, although not inclusive are considered examples of Support Coordination/Case Management functions:
Assisting with orienting the summer to his/her support services
Completing an annual comprehensive assessment (unless significant changes occur in the person's life which would require a new assessment prior to the review due date)
Meeting with the consumer of a face-to-face basis to promote their participation in their plans, goal, and status. (Individual needs/desires will determine the frequency of additional contacts)
Linking consumer to support services - generic, community and/or within OCCMHA
Assisting with financial management and coordination
Assisting with coordination of health and safely accommodations and strategies
Assisting with development of a consumer's social support network
