Supportive Services Information Center

Community Housing Network, Inc. is pleased to provide the Supportive Services Information Center to assist consumers, family members and Supports Coordinators with information about available supports and how to access them.

Supportive services are the services a person needs to live independently in a home. These can range from assistance with chores and bill paying, to case management, to the 24/7 staffing someone with high needs would need to live, whether in a group home or independently, in an apartment or home.

In home supports are paid for through adult home services from the Family Independence Agency and Medicaid, which is administered through the Oakland County Community Mental Health Authority and subcontracted to the core providers. Medicaid pays for mental health services which may include supports coordination and must include access to a Person Centered Plan for Medicaid eligible individuals.
